The narrow holes are just for decorative effect. It is the broad holes down the centre where the ribbon goes. The bobbin count is for a single twisted passive either side of the broad hole. If you want to have cloth passives instead (which would give a stronger and more emphatic edge) then the bobbin count becomes 24 pairs.
The scallops above the hearts are not standard ones, and the centre is rather unsupported. It does not seem to matter, though.
There is not a particularly pretty start to this pattern. I have given a couple of false pins for the start. It might be best to do a couple of rows across all other pairs using one of the edge pairs before starting the pattern, as this will give a stronger start.
